How To Initiate an AT Assessment

1) Go to the Assessment section in the AT Corner of the KCSEC website
2) Fill out the Referral/Question ID Guide form for the student.
3) Fill out the Student Information Guide that is applicable for the student to be assessed. For example, if the student is having difficulty writing legibly, then fill out the Student Information Guide titled “Motor Aspects of Writing”.
4) Fill out the Student Information Guide titled “General”.
5) Fax or email the completed forms to your coordinator and the assistive tech specialist assigned to your building.
